Getting Started

Your relationship with Lakeshore Clinic begins with these few simple steps. Please take a moment to review this section to answer any questions you may have.

Registration and Appointments
We want our patients to see their provider as soon as possible. Our clinics are open extended hours for your convenience. We also have Saturday appointments. To schedule an appointment please call either our Totem Lake or Bothell Clinic.

If you are a new patient you can save time by completing our Patient Registration Form (Adult/Child) and Patient History Form (Adult / Child) prior to your visit. Please remember to bring a photo ID and your insurance card. For your first visit please come in 15 minutes early.

Please have the following information available to schedule an appointment or procedure: full patient name, date of birth, which provider they will need to see, and the nature of the patient's visit.

All patients receive a reminder call 2 days prior to their appointment. If you must cancel an existing appointment, please call at least 24 hours prior to the scheduled time of your appointment.

A "no-show" occurs when a patient fails to show for a scheduled appointment or cancels without 24 hours notice. No-shows may be subject to a fee as directed by the provider. The first no-show the patient will receive a letter outlining the policy and notifying them of our fee. Upon the second no-show the patient will receive a letter notifying them that their account has been charged the fee.

All co-payments are due at the time of service. If not paid at the time of service, you will be charged an additional $20.00 missed co-payment fee.

Checks are only accepted from established patients.

*ACCEPTING ESTABLISHED PATIENTS ONLY. Established Medicare Patients must have approved Medicare Advantage Plans. See Medicare Changes.

We are not contracted with any discount plans even if they have any one of the above listed health plan logos on them.

No Healthy Options plans of any kind.

Any card that shows a PCP (Primary Care Physician) other than Lakeshore Clinic Providers require prior written referral to be seen.

Some plans do not show logos, so if you are unsure please call our Business Office at (425) 485-3955 or the number listed on your insurance card.

We only bill MVA's (Motor Vehicle Accidents) if it is the patient's PIP (Personal Injury Protection) coverage; otherwise it is the patient's responsibility to pay the bill in full or setup subrogation with their medical carrier.

An NSF fee of $35.00 is charged for all NSF (returned) checks. In the future you will have to pay with cash or credit card only.

For your convenience we do accept Visa and MasterCard. Checks are only accepted from established patients.

Please contact your Health Plan directly if you have questions regarding eligibility or benefits, as this is a contract between you and your Health Plan.
